Images have a Box mode and an Img mode, which you can switch between using the toggle at the top right of the selection frame.
To insert a child box such as text, make sure the image is set to Box mode and that its size is larger than the child element.
In Img mode, the image's aspect ratio is preserved, but you can't add child boxes or apply filters.
