The main causes of form submission failures are Google reCAPTCHA v3 configuration issues or Google Sheets integration issues. You can resolve them by following the steps below.

How to Check and Remove Google reCAPTCHA v3 Integration

  1. Open the dashboard for your project.

  2. Open the Apps menu and check whether "Google reCAPTCHA v3" is configured.

  3. If it's configured, remove the integration temporarily.
    Clear the "Site Key," "Secret Key," and "Threshold" fields, then click [Save].

  4. Try submitting a form response and check whether "Form Submission Successful" appears.

  5. If "Form Submission Successful" appears, review your Google reCAPTCHA v3 settings.

How to Disconnect and Reconnect Google Sheets Integration

If you've linked your form responses to Google Sheets, try the steps below.

Data submission may stop if the sharing permissions or Google account information for the spreadsheet have been changed.

Steps

Disconnecting and reconnecting the integration will generate a new sheet that updates automatically.

  1. Open your project and go to the Form Dashboard.

  2. Select the form you want to disconnect and open its settings.

  3. Click [Disconnect].

  4. Click the external integration icon.

    Grant access on the screen that appears. Once the integration is complete, a dedicated sheet will be automatically generated, and it will update automatically whenever new responses come in.

  5. Try submitting a form response and check whether "Form Submission Successful" appears.
